Carotid Endarterectomy









     A patient presented with sudden onset painless complete monocular loss of

     vision of the right eye for a few seconds, followed by complete spontaneous

     recovery. This is amaurosis fugax and is due to emboli to the ophthalmic
     artery (first branch of the internal carotid artery) from either the carotid

     artery or from the heart.



     He had normal sinus rhythm with a normal ECG and echocardiogram, and a

     carotid duplex showed 70% stenosis of the distal right common carotid artery

     extending to the internal carotid artery.



     Right carotid endarterectomy was offered as a prophylactic procedure for

     further neurological event. The patient was started on anti-platelet therapy in
     the form of  aspirin.
