Saccular Arch Aneurysm
A 71 year old man presented with hoarseness and CXR showed a large mediastinal shad-
ow. Subsequent CT scan confirmed a large 7cm saccular aortic aneurysm located in the
arch, compromising the L recurrent laryngeal nerve and L cord palsy.
Traditional open surgery and total arch replacement carries a high risk of mortality and
stroke, and often declined by patients who are elderly with significant morbidities. Mod-
ern endograft technology allows endovascular repair of the aortic arch, in this case with
the Terumo Double Branch device.
