This was confirmed by another CT scan of the thorax. The arch aneurysm
measured 6cm diameter and had an ugly penetrating ulcer proximally.
Endovascular treatment would have to be a very marginal arch branch

device with limited proximal landing zone. This is also a large device (>9Fr)
and the calcified iliac arteries will pose access difficulties.
