A 93-years old lady with multiple co-morbidities, bedridden and on nasogastric tube feeding was
found to have these gangrenous patches over her toes. There was not resting pain and her feet was
warm, and veins are filled. All her lower limb pulses were not palpable and the femoral arteries felt
calcified bilaterally. This is likely a result of localized pressure necrosis with no evidence of infection.
In view of her comorbidities and age this was managed conservatively as revascularization is proba-
bly not going to be beneficial.
