The Perm-cath was placed through and through the right internal jugular vein at the
clavicle level and has entered the right chest cavity through the mediastinum, confirmed
on this CT. The patient developed a right hemothorax and the catheter had to be re-
moved with thoracoscopic guidance.
We are seeing more of these iatrogenic injuries. Clear knowledge of anatomy and a
high sense of suspicion when something is not right is the key to avoid these complica-
tions.
