1. Once the aortic aneurysm sac was
opened, turbid bile stained fluid was
found, confirming an aorto-
duodenal fistula. The previously
placed EVAR stent graft was
exposed.
2. The fistula opening connecting the
aneurysm sac and duodenum can be
clearly visualized.
3. The aneurysm sac was retracted
laterally exposing the duodenal
opening. Necrotic edges were
trimmed before contemplating a
primary repair.
