Vasospasm

A 78 year old patient with a chronic history of SLE, Raynaud’s syndrome, renal impairment, Rather dramatic clinical improvement after overnight treatment with LMW heparin and
congestive heart failure and coronary artery disease presented with a two week history of warmth. She was also given a low dose iliprost (prostaglandin) which may have a risk of pre-
severe cyanosis of her left foot in cold weather. There is some early gangrenous changes in cipitating hypotension. Its use in patients with a poor heart should be balanced against poten-
the tip of the big toe. Her popliteal pulses were strong. These are clinical suspicions that this tial benefits.
could be due to cold related vasospasm and may be reversible.
