ANATOMY
A 73-year old man suffered from acute myocarditis presenting with shock. ECMO support
was needed (R femoral cannula) and an emergency PCI (percutaneous coronary intervention)
was performed by cardiologists via the left side. The 7Fr left percutaneous “femoral” sheath
was obviously placed too high and above the inguinal ligament (arrow) instead of via the left
common femoral artery below. The sheath went intraperitoneal into the left external iliac
artery. Fortunately the patient did not suffer bowel perforation, but he died from ischemic
bowel as a result of shock.
