A CT scan confirmed a saccular aneurysm of the proximal descending thoracic aorta. This appear-
ance is suggestive of a mycotic pseudoaneurysm. The primary treatment would be endovascular
stent grafting although there is a risk of persistent infection. The alternative of open surgical repair
would be considered too high risk. The patient should be given a course of antibiotics with workup
for bacteremia. Note also the “Bovine” origin of the innominate and L common carotid arteries.
