The choice of treatment included:
1. Laparotomy and splenectomy. This is the most direct and certain way of treating a
ruptured aneurysm, and should be well tolerated in a young patient.
2. Coil embolization: Minimally invasive, yet the success for an aneurysm of this size
was uncertain. Patient has some risk of continuous slow bleeding leading to eventual
laparotomy, complications of DIC and transfusion.

The surgeons chose to try coil embolization by radiologists. The pre- and post-emboliza-
tion angiogram were shown above. With a combination of coils in the proximal splenic
artery and glue, the inflow into the sac seemed to be reduced. No coils could be placed
inside the aneurysm tough.
