Mycotic AAA
A 63 years old male presented with back pain for more than one year. The sudden
onset of back pain occurred January last year, he went to see orthopedics, neurol-
ogist several times for the pain in the past year. He had a non-contrast CT scan in
February last year at another hospital, but the aortic pathology was missed.. They
just focused on the spine, and gave conservative treatment.
He came to a pain clinic for the back pain and MRI scan and then contrast CT has
been done. The mycotic AAA was larger compared with the CT image last Febru-
ary, and the lumbar spine body was eroded. He has no fever with an elevated WBC
of 14.45, CRP 22umol/L. Blood culture was negative. Final diagnosis was aortic
infection with Coxiella burnetii (Q-fever)
