Stent Graft Infection

































     CT scan showed the endovascular repair was intact, but there were gas pockets inside the aneu-
     rysm sac around the stent graft and a bulge outside.  This appearance is highly suggestive of an
     infective process and is a dreaded long term complication of endovascular repair.  In this old and

     frail gentleman there are no viable surgical options.  He is likely to die from persistent sepsis and
     pneumonia despite antibiotics.

 This frail 85-year old man had a aortic arch branched stent graft one year ago for a large arch an-
 eurysm.  The branch stents were to the innominate and the left subclavian artery with an added
 L carotid-subclavian bypass to revascularize the left carotid system.  He presented one year later
 with fever and cough.
