This was surgically explored and the haematoma evacuated. The skin was trimmed, and wound

 closed with a drain in situ.

                 61-year old patient with diabetic nephropathy, renal transplant presented with
             A this dry necrotic patch after self-inflicted injury during nail cutting.  His vessels
             are calcified with some common femoral and SFA and distal disease.  The toes are
             however well perfused.  A period of conservative treatment may be worthwhile
             instead of immediate revascularization.
