Mechanical Thrombectomy

A 68 years old male presented with right foot pain, cold and numbness for 10 days, without
a past history of AF. CTA showed right mid SFA occlusion. Acute thrombosis on chronic SFA
stenosis was highly suspected based on his symptoms, ultrasound and CT. Ultrasound guided Intraop Rotarex (left) and after thrombectomy (right)
antegrade right femoral puncture, 0.035 guidewire successfully passed the occlusion, although
some difficulty encountered at the distal end of the lesion (a calcified plaque was noted at
this location on preoperative CT). The guidewire was ensured to be in the true lumen, Rotarex
thrombectomy was performed. Multiple stenosis was noted after Rotarex thrombectomy,
4mm and 5mm balloon were used for angioplasty. And a 5mm DCB (Acotec) was used. Satis-
factory flow was seen on completion angiogram. No stent implantation. (SZH)
