AAA with Abdominal Pain
A 90-year old man with renal impairment, recent STEMI (on double anti-platelets), known
abdominal aortic aneurysm (6cm), unresectable CA liver, declined all surgical treatment. He
sustained a fracture L hip after falling. One week after L hip arthroplasty he complained of
severe R sided abdominal pain, hypotension, and Hb dropped to 5.9g/dl.
In all patients who had an AAA and complained of abdominal pain a leaking/ruptured an-
eurysm must be considered. The clinical signs of a tender R abdomen and frank bruising
(Grey-turner sign) is indicative of retroperitoneal hemorrhage.
