A 87-year old man presented to the Accident and Emergency Department complaining of dif-
fuse abdominal pain. He had a large AAA which was missed by the examining officer. He was
sent home only to return 24 hour later with increasing abdominal and back pain. CT scan con-
firmed a 7.5 cm juxtarenal AAA with evidence of retroperitoneal leak. The aneurysm is large
with no neck and not suitable for endovascular repair. Emergency open repair was performed.
