Axial CT showing the same finding. An endovascular repair with a fenestrated
graft was considered but deemed very difficult due to the angulation of the R re-
nal artery, the narrow “waist” of the aorta below the R renal origin, and multiple L
renal arteries would make this very challenging.
This 82-year old man has a large pararenal AAA of 8cm in diameter.
Note that there is no healthy “neck” below the SMA and also the
presence of an additional, lower left renal artery. This is often the
case when an AAA was discovered only when it reaches a very large
size.
