CTA showed bilateral common carotid occlusion, extensive neck artery calcifications, with
          the brain supplied by a dominant L vertebral artery.


          Treatment of occluded common carotid, usually RT induced, is very difficult since stenting
          is no longer an option.  This patient has no useful inflow vessels accessible in the neck.
          Surgical bypass may be a left subclavian to right carotid bypass but would be difficult due
          to an irradiated field in the neck, and diseased thickened arteries adding to the risks of
          intraoperative stroke.
