urgical revascularization was attempted by a private surgeon. Repeated attempts to
Sthrombectomise the superficial femoral and popliteal were not successful and outflow was
compromised. A stenotic profunda femoral artery was not treated. Attempts to improve the
inflow with embolectomy of the iliac system was unsatisfactory. This was followed by balloon
angioplasty and a stent in the iliac artery (failed, dissected). Finally an aorto-left femoral bypass
was constructed but this also failed due to narrow anastomosis and poor quality common femo-
ral arteries. Note the profunda femoral artery was occluded which is a serious sign.
With no inflow and no outflow the patient’s left leg deteriorated rapidly into very critical is-
chemia with mottling, paralysis and pre-gangrenous changes.
