his is a very critically ischemic leg with delayed presentation exhibiting all the signs of acute
Tischemia. The foot is cold, cyanotic with mottling, paralysed (unable to move the toes and
ankle), with collapsed veins. The skin color changes extended to the anterior compartment of
the calf which is also tender. The foot is very marginally viable and would have been lost with
another few hours of delay. The symptoms of acute ischemia is completely different from chron-
ic ischemia (ulcers and gangrene) and the urgency should be recognized.
Emergency exploration was performed right after admission. Inflow and outflow was re-estab-
lished.
