Due to his advanced age and severe stroke
and hemodynamic instability, open surgical re-
pair was not offered. The patient died shortly
after admission.
Tradition surgical treatment for Type A dis-
section is still open surgery under cardiopul-
monary bypass. Options included ascending
aortic replacement with / without aortic valve
replacement, and hemi-total aortic arch re-
placement with or without a frozen elephant
trunk. There is a significant risk of mortality
and morbidity including stroke and organ
damage.
Acute Type A dissection carries an initial mor-
atlity of one percent each hour, with a hospi-
tal mortality exceeding 50%.
