For deep dermal and full-thickness burn wounds,
surgical debridement and split thickness skin
graft is warranted.
Tangential excision is the commonest procedure
performed for burn wound debridement. It
involves sequential debridement of necrotic
tissue till healthy and well-vascularized tissue is
encountered.
In contrast, fascial excision is typically reserved
for patients who cannot tolerate excessive
blood loss and prolonged operating time
associated with tangential excision e.g.
unstable patients with extensive areas of deep
burn wounds. It involves excision down to deep
muscle fascia. Major drawbacks include loss of
cutaneous sensation, lymphedema and tissue
contour deformities.
