Aortic Anatomy
This gentleman was found to have an 8cm AAA. The aorta was extremely tortuous
and sigmoidal shape. Endovascular repair is contraindicated in such extremely tortu-
ous aorta.
Lesser tortuosity may be ‘corrected’ intra-operatively to an extent by the use of intra-
luminal stiff guidewires to straighten the aorta prior to stentgraft placement. However,
these guidewires can also cause iatrogenic aortic perforation if used incorrectly. This
would not be realistic in this sort of extreme tortuosity.
