Surgical options include open carotid endarterectomy with concomitant retrograde angioplasty/
stenting of the left CCA origin or stenting of both lesions. In this patient, we performed stent-
ing/angioplasty of both lesions through retrograde and antegrade approach via a small left neck
incision. Throughout the procedure a filter wire was placed at the distal ICA for cerebral protec-

tion. The intraoperative angiograms are shown- ICA stenosis, ICA post stenting, CCA origin ste-
nosis, CCA post stenting.
