After ascertaining microscopic tumour clearance on frozen section, the size of the defect
was measured. Thin flaps such as the free radial forearm and posterior tibial fascio-
cutaneous flap could be used to reconstruct the buccal mucosal defect.
Free posterior tibial flap was harvested from the medial surface of the lower leg. The two
perforators (marked with “x”) stem from the posterior tibial artery and vein. Vascular
clamps were applied to the pedicle artery and two venae comitantes on flap harvest.
